Fixes #4770.

What Changed

The context-window tooltip now resolves its compaction label from the active thread's exact provider instance and selected model. It uses the same short display name as the model picker, falls back to the selected model slug when metadata is unavailable, and uses neutral copy when there is no model selection.

A focused regression test covers multiple instances of the same driver, including two instances that expose the same model slug with different display metadata.

Why

The tooltip previously found the selected provider instance and then passed only its driver to getProviderDisplayName. That driver-scoped lookup resolves the default/first instance, so a thread using another Codex profile could show an unrelated instance name such as Yufei Pro 20x.

Describing compaction with the selected model is accurate and matches the model picker already visible beside the context meter. This keeps the change scoped to presentation logic; context accounting and provider behavior are unchanged.

One finding: the compaction hint in ContextWindowMeter swaps the themed text-secondary-label token for text-muted-foreground/70, which bypasses the custom-theme token bridge and lowers contrast. Details inline.

Reconciles 101 upstream commits (184d8ef..20a7042) into the fork trunk.
337 files, +13,062/-2,558. The largest reconcile since the connection rewrite,
and the one with the most competing implementations so far.

Four upstream changes landed on ground the fork had already built, and each was
resolved toward the fork's superset rather than by picking a side:

- pingdotgg#4781 "keep send reachable while a turn is running on mobile" converged on the
  fork's own Send-beside-Stop: same removed early-return, same hoist into a
  `sendButton` const. Upstream gates it behind a mobile-only prop; this fork
  shows Send on every viewport, backed by the server-side FIFO queue. Kept the
  fork's arrangement and REMOVED the now-vestigial showSendWhileRunning prop
  rather than leaving dead API. Upstream's three tests for the gated semantics
  are deliberately absent, with a comment, like the steering test.
- pingdotgg#6773 "collapse the question prompt from its header" reimplements the fork's
  ask-question collapse on the Collapsible primitive, without the fork's
  bounding (max-height + internal scroll). Kept the fork's. Upstream's pingdotgg#6646
  multi-select fix in the same file is orthogonal and WAS taken: the handler is
  now a useCallback rather than an effect event.
- pingdotgg#4083 "show command output in work log" adds an inline expanded body to a
  work-entry row; this fork opens that detail in a modal and had already dropped
  upstream's stopRowToggle for that reason. Rejected the block and its two
  helpers, but kept toolCallExpandedBodyClassName, which upstream's tests import
  and which carries pingdotgg#6510's code-font sizing.
- pingdotgg#6636 "accept file drops across the chat workspace" MOVED drop handling from
  the composer to the workspace, so its empty side of that conflict was a
  deletion, not an absence. Adopted upstream's architecture and put the fork's
  image/non-image split inside addDroppedFiles: a non-image dropped anywhere in
  the workspace now inserts its path, wider than the composer-only target.

Two more were complementary rather than competing, and both halves were kept:
pingdotgg#6174 resolves a diff path when the project root is NESTED inside the
repository, while the fork's repoCwd records WHICH repository a surface belongs
to; and pingdotgg#4133's parseRawHtml gate now wraps only the raw-HTML rehype pair,
leaving the fork's file-path plugin always on.

ContextWindowMeter.tsx was a delete/modify conflict: the fork deleted it when
the Vitals gauge replaced it, and upstream is still editing it (pingdotgg#4772). Kept the
deletion, dropped the composer's re-added import and usage, and removed the now
orphaned ContextWindowMeter.logic pair. pingdotgg#4772's model-name-in-tooltip is
therefore not ported; the gauge shows the provider name instead.

Invariants: all six re-probed against the merged tree and clean. Migrations 44
entries, unique, monotonic, max 45, only gap 34. The FORK-ONLY
@effect/platform-node patch survives (effect stays at beta.103, no retirement
due; upstream still carries no patch of its own for it). Sidebar default still
v2. Both project-row buttons intact — and upstream's Button restyle was adopted
for the settings one, with ml-auto left on the ellipsis so the two do not fight.
The steering test is still deliberately absent.

Sweeps: 0 resurrected. 112 dropped, every one traced to a decision above.

The gate went red first and caught two things no conflict showed: upstream
trimmed getProviderDisplayName from an import the fork still uses, and its new
command-lifecycle test waited on a runtime.warning that this fork emits as its
own runtime.notification. The first was a one-line restore; the second was
retargeted rather than deleted, since pingdotgg#6606's actual subject still applies.

Gate green: typecheck, lint, 9,109 tests across 14 projects, 909 files, 0
failures, on a full install whose lockfile matches the committed one.

The context block showed a window size with no indication of which model it
belonged to — ambiguous on a fork where the active model changes per thread. The
header caption now reads "Opus 5 · 1m window".

This closes the follow-up the 101-commit reconcile recorded rather than dropped
silently. Upstream pingdotgg#4772 put the model name in ContextWindowMeter's tooltip; the
fork deleted that component when the Vitals gauge replaced it, and the reconcile
removed the orphaned ContextWindowMeter.logic.ts along with it. Rather than
revive a module whose only consumer is gone, the ~6 lines of resolution are
reproduced next to the existing provider-name memo, using the
getTriggerDisplayModelName helper that survived. Falls back to the raw slug for a
custom or since-removed model, matching upstream.

The caption is gated on EITHER half rather than on the window size: a provider
that reports no maxTokens is exactly the case where naming the model matters
most, and hanging the name off the size's existing hasMax gate would have
dropped it precisely there. That was a design-review finding, and it has a test.

Rendered in the header's existing right-hand slot, so it costs no vertical space
in a popover that already carries three blocks — rather than upstream's tooltip,
which would put a second hover layer on a surface that is itself a disclosure.

Gate: typecheck, lint, 9,117 tests across 14 projects, 0 failures.

  errors and schema decode failures. Pre-existing in HEAD before this
  merge, just never exercised until the full test suite ran clean of
  conflict markers.
- packages/contracts/src/settings.test.ts: removed a test asserting that
  decoding drops the sidebarV2Enabled/sidebarV2ConfiguredByUser keys —
  contradicted the adjacent "ClientSettings sidebar v2" tests and the
  real schema, which keeps both fields alive alongside legacySidebarEnabled
  for BetaSettingsPanel.tsx.
- packages/contracts/src/settings.ts: fixed the legacySidebarEnabled
  comment to describe the fields as coexisting, not superseding.
- apps/web/src/browserFaviconStore.test.ts, browserHistoryStore.test.ts:
  switched their `~/state/session` mocks to importOriginal() + override,
  so the real environmentSession export (now used transitively via
  state/server.ts) stays available instead of being dropped entirely.

Full repo-wide `vp run -r test` passes (aside from two known-flaky
poll-based tests that pass in isolation but time out under full
concurrent load, unrelated to this merge).

`prepareCommandCenterCodexHome` only copied `auth.json` into the isolated
home when the source home had one, and never checked the result. A source
home without credentials produced a working session that failed on its
first model call with an opaque provider 401 naming neither the missing
file nor the home Command Center actually read.

Require credentials in the isolated home before returning the layout, and
name the resolved source path in the error.

Seed `auth.json` in the fixtures that relied on the previous fail-open
behavior; they cover home layout and profile injection, not auth.

Codex 0.147 asks to widen its sandbox with
`item/permissions/requestApproval`. No handler was registered, so the
request fell through to `handleUnknownServerRequest` and was answered
with a JSON-RPC "method not found". Codex reports that as
`user rejected MCP tool call`, so the turn failed with an approval the
user was never shown and could not answer.

Register the handler and map the method through to a real approval
prompt. The request carries an arbitrary path list (each read/write/deny)
plus a network toggle, so the decision is classified on the requested
permissions rather than on the tool that triggered them — a read-only
tool can still ask for write.

`autoApproveReadOnlyPermissions` (off by default) grants an escalation
without prompting only when every entry is a read: any write, sandbox
denial, legacy write list, or network enable still prompts. Auto-grants
are scoped to the turn, never the session.

The approval channel carries only accept/decline, so the granted profile
is the requested one echoed back on accept and an empty profile on
refusal, mirroring how ClaudeAdapter replays SDK permission suggestions.

Requests are surfaced with the `file-change` kind: ingestion derives the
kind from the canonical request type alone, and a kind that varied per
request would disagree between the opened and resolved activities. A
request with no kind renders no prompt in web or mobile while the server
still opens a pending row, which parks the thread unanswerable.

The persisted activity keeps only `detail`, so the requested paths are
folded into it — otherwise the prompt cannot be reviewed.

'--filter=...'`, started no dev server, and exited 0. The launcher reported
success while the stack was already gone.

Pin the repo's node_modules/.bin ahead of the inherited PATH so `vp` always
means this repo's toolchain, whatever environment we were launched from.

stale fork-only .ts copy.
