Require form data by default

This PR:

• Fixes form data not being marked as required by default (fix MultiPartFormFileSchema defaults to required=false #762)
• Add documentation around query parameters being optional by default (minItems not work with empty array in query params #949)
• Fix incorrect Go version change from previous PR (Fix various issues #961)
