I encountered a session rate limit error in VS Code while using Copilot/Chat (GPT-5.3 Codex), even though my premium request usage is only around 50%.

Environment:

• Editor: VS Code
• Feature: Copilot Chat / Agent mode
• Model: GPT-5.3 Codex
• Usage shown: ~50% premium requests used (not exhausted)

Issue:
While working normally, I received the message:
“You’ve hit your session rate limit. Please upgrade your plan or wait for your limit to reset.”

This seems inconsistent because:

• Premium requests are not fully used
• No intentional heavy or bulk requests were made
• Work involved standard development tasks (file edits, reviewing code, small prompts)

Possible Causes (Suspected):

• Background/implicit requests (e.g., file watching, auto context updates, agent loops)
• Session-based limits separate from premium request quota
• Increased hidden usage due to multi-file context or live UI monitoring

Questions:

1. How does the session rate limit differ from premium request limits?
2. What internal/background actions count toward this limit?
3. Is there a way to monitor real-time request consumption or debug usage spikes?

Expected Behavior:
The session should not hit limits prematurely when premium usage is still available and usage pattern is moderate.

Impact:
Interrupts development workflow and prevents continued use despite available quota.
